Summary

This study tests a home-based cardiac rehabilitation program delivered through a custom mobile app for people with heart failure. About 100 participants will be randomly assigned to either the app-based program or usual care. The goal is to see if the app helps increase physical activity, improve quality of life, and reduce heart-related events.
