Summary
This study is testing whether having extra appointments with a heart specialist at your local doctor's office leads to better management of health risks like high blood pressure and cholesterol. It compares this approach to standard care from a general practitioner. The goal is to see if the specialist-led care helps patients get treatments more in line with national health guidelines, which could lower their future risk of heart attack, stroke, and other problems.
