Summary
This program aims to reduce health inequities by connecting people in Massachusetts with support for three key health areas: cancer screening, weight management, and mental health. It will work with local clinics to offer evidence-based help to adults who are overdue for screenings, have obesity, or experience depression/anxiety. The goal is to improve access to care and health outcomes in communities that often face barriers.
