Summary

This study is testing whether using a continuous glucose monitor (a small sensor that tracks blood sugar levels every few minutes) can help people who have both type 2 diabetes and heart failure. Researchers want to see if better, real-time sugar monitoring during and after a hospital stay can improve diabetes control and reduce the chances of being readmitted to the hospital for heart problems. They will follow 140 adults for three months after they leave the hospital.
