Summary
This study looks at whether a detailed health check-up, called a comprehensive geriatric assessment, can be used in regular doctor visits for people aged 65 and older. Researchers will track how frailty changes over a year and compare patient self-reports with doctor assessments. A smaller part of the study will test if a personalized exercise program helps improve physical activity and frailty. The goal is to learn how to better support healthy aging in primary care.
