Summary
This study looks at whether a detailed health check-up, called an electronic Comprehensive Geriatric Assessment (eCGA), can be used in regular doctor visits for people aged 65 and older. It will track how frailty changes over a year and compare how patients and doctors rate health. Some participants will also get a personalized exercise program to see if it helps improve strength and activity levels. The goal is to find better ways to support healthy aging in primary care.
