Summary
This study enrolls 200 adults aged 50 and older in a rural area who have had a hip or spine fracture or are on osteoporosis medication. It provides education, monitoring, and follow-up every 4 months for 2 years to help patients stick with their treatment and reduce fracture risk. The goal is to improve medication adherence and bone health through a coordinated care service.
