Could a simple clinic Check-In change catch hidden high blood pressure and diabetes?
NCT ID NCT06406192
First seen Jul 24, 2026 · Last updated Jul 24, 2026
Summary
This trial tests whether improving how clinics measure and record blood pressure, height, and weight can help identify people with undiagnosed hypertension or diabetes. About 800 adults visiting family medicine clinics will be part of a study where some clinics use the FOR-Care model—with nurse assistance, better equipment access, and reminders—while others continue usual care. The goal is to see if this simple change boosts detection of these common chronic conditions.
What this could mean
Our plain-language read of the trial. This is informational only — not medical advice or a prediction.
- Active substance
- FOR-Care model: improved measurement and recording of blood pressure, height, and weight in outpatient clinics
- What this could lead to
- If it works, this simple change in clinic routines could help catch more cases of high blood pressure and diabetes early, improving preventive care.
- What could go wrong
- The trial tests a process change, not a new drug, so the impact may be modest. Results depend on clinic staff and patient participation, and may not apply to other settings.
