Summary

This study tests a program where doctors' offices help families who struggle to afford food get connected to community nutrition support. About 600 families with children aged 5-18 who have certain health conditions and receive Medicaid or SNAP will take part. The goal is to see if this approach is practical and helps improve food security and health over 12 months.
