Summary
This study tests whether a nurse care management program can help 450 rural adults with chronic pain feel better and function more in daily life. The program includes care coordination, cognitive behavioral therapy, and a remote exercise program. Participants must live in a rural area, have chronic pain for over 3 months, and be a current primary care patient.
