Summary

This study is testing a new system to help cancer survivors manage their weight through exercise. Doctors will use a simple 3-step plan (assess, advise, refer) to connect eligible patients with a weight management program. The goal is to see if this structured approach gets more survivors the exercise support they need compared to usual care.
