Summary
This study looks at whether a telehealth program called ECHO can help healthcare teams in New Mexico better care for patients with diabetes and other chronic conditions. The program connects primary care providers with specialists through video sessions for training and case discussions. Researchers will compare blood sugar levels in patients whose healthcare teams received the training versus those who did not. The goal is to see if this approach leads to better blood sugar control over 18 months.
