Summary

This study looks at whether a simple frailty score (ISAR) can identify older adults in the emergency department who are at risk for medication problems, like harmful drug interactions or inappropriate prescriptions. Researchers will collect medical data from 300 patients aged 75 and older, without changing their care. If the score works, it could help pharmacists focus on the patients who need medication reviews the most.
