Summary
This study looks at whether a simple frailty screening tool (ISAR) can predict medication problems in elderly patients visiting the emergency department. Researchers will review the medical records of 300 patients aged 75 and older to see if those with higher frailty scores are more likely to have risky prescriptions or drug interactions. The goal is to give pharmacists a quick way to prioritize which patients need a medication review, without changing any patient care.
