Summary
This study is testing whether a doctor-led lifestyle counseling program, combined with two personalized smartphone apps, is practical and helpful for older adults who have received a liver or kidney transplant. Participants first receive 12 weeks of medical advice on healthy eating and physical activity, then use the apps at home for another 12 weeks while researchers track their app use and health changes. The main goal is to see if this combined digital and counseling approach is feasible for this group and if it can improve their diet, activity levels, and related health measurements.
