New program aims to help young adults navigate diabetes care

Summary
This study is testing a new support program for young adults (ages 18-30) with type 1 or type 2 diabetes as they move from pediatric to adult healthcare. Researchers will compare this new program to standard care to see if it helps people keep doctor's appointments and reduces the stress and anxiety that often comes with managing diabetes. Participants will fill out surveys and some will be interviewed to help improve the program for the future.
