Summary
This study tests whether community health workers can help people better manage multiple chronic conditions like diabetes, high blood pressure, and heart disease. 300 adults with at least two chronic health conditions will work with trained health coaches for 9 months to develop personalized health plans. The program focuses on helping participants understand their conditions, navigate healthcare systems, and build confidence in managing their own health.
