Summary

This study tests a community-based care bundle for treating and preventing pressure injuries (bedsores) in older adults at home. 216 participants discharged from Singapore General Hospital will either receive the bundle (including special cushions, skin barriers, and caregiver training) or routine care. The goal is to see if the bundle improves healing and prevents new injuries over six weeks.
