This study looks at whether giving older adults (65+) with diabetes or high blood pressure access to their visit notes and audio recordings of their appointments helps them feel better and manage their health. About 1,022 participants will be randomly assigned to receive either written notes through a patient portal or notes plus audio recordings. The goal is to see if these tools improve quality of life and self-care.
