Summary

This study compares two ways of tracking blood sugar in people with type 2 diabetes who use insulin after leaving the hospital. One group uses a continuous glucose monitor (CGM) that shows real-time levels and trends, while the other group uses traditional fingerstick tests. The goal is to see if CGM leads to better blood sugar control over 12 weeks, with follow-up at 24 weeks. 160 adults aged 18–80 with type 2 diabetes and an HbA1c between 8% and 13% are being recruited.
