This study tests whether a diabetes care program using continuous glucose monitors (CGM) and remote patient monitoring can help adults with uncontrolled type 2 diabetes improve their blood sugar levels and reduce diabetes-related distress. Participants will wear a CGM that shows how their glucose responds to food, activity, and medication, while their care team remotely monitors the data and adjusts treatment as needed.
