Summary

This study is testing whether wearing a continuous glucose monitor (CGM) after hospital discharge helps people with type 2 diabetes better manage their blood sugar levels. The research follows 108 hospitalized patients with type 2 diabetes who wear a Dexcom CGM for 12 weeks after going home. The study measures how much time patients spend in their target blood sugar range and how satisfied they are with using the monitor.
