Summary
This study tests a structured tool called CSNAT that helps family caregivers identify their own support needs and have guided conversations with a nurse practitioner. About 80 caregivers of older adults receiving home-based primary care will either use the tool or receive usual care. The goal is to see if the tool is practical and acceptable, and whether it can reduce caregiver strain and improve well-being, which might also lower the risk of elder mistreatment.
