Summary

This study tested a dementia care program delivered by phone and web, called the Care Ecosystem, in 6 health systems across the U.S. Over 1,200 people with dementia and their caregivers took part. The program used trained navigators supervised by dementia experts to improve care quality, reduce emergency room visits, and lower caregiver depression. The goal was to see if this model works in real-world settings, not just in controlled research.
