Summary
This study tests a program called CARE-D-Foot-Nav, where a diabetes educator acts as a personal navigator for people with diabetic foot ulcers. The navigator helps with blood sugar control, wound care, transportation, and emotional support. Researchers want to see if this extra help leads to more ulcers healing within 20 weeks compared to usual care alone. The study plans to enroll 270 adults with diabetes and a foot ulcer.
