Summary
This study is testing a new care model called OPTIMISE, designed to improve overall health for cancer patients who also have other chronic conditions like diabetes or high blood pressure. It focuses on better communication and coordination between a patient's cancer doctor and their primary care doctor. The goal is to see if this team-based approach helps patients manage their health more effectively and improves their quality of life during and after cancer treatment.
