Summary
This study is testing if adding a special home-visit care team to standard Medicare wellness visits improves care for older adults with multiple health problems. The goal is to see if this combined approach leads to better patient and caregiver experiences and fewer hospital stays. The study will enroll over 6,000 older adults and their caregivers to compare the two approaches.
