New teamwork model aims to fix gaps in cancer survivor care

Summary
This study is testing a new way to coordinate care for cancer patients who also have other ongoing health problems like diabetes or high blood pressure. It will involve 340 patients with breast, gastrointestinal, or blood cancers. The goal is to see if better teamwork between cancer doctors and primary care doctors, guided by a nurse navigator, helps patients manage their overall health and improves their quality of life.
