Summary

This study tests an automated system that uses patient risk levels to send reminders for follow-up appointments. About 1,000 adults with inflammatory bowel disease (IBD) will be randomly assigned to either usual care or the reminder tool. The goal is to see if the tool reduces missed visits and improves outcomes like fewer emergency visits or steroid use.
