Summary
This study is testing a new support program for people with Alzheimer's or dementia and their caregivers during the transition from a nursing facility to home or another care setting. The program provides extra help and education to nursing facility staff and assigns a specialist to support families after discharge. Researchers want to see if this reduces stress for caregivers and improves behavioral symptoms for patients.
