Summary
This large study tested a program to help nursing home residents with Alzheimer's and their families plan for future medical care. The goal was to reduce stressful and unwanted hospital visits by making sure a patient's treatment preferences are clearly known and respected by the care team. Nursing home staff were trained to have better conversations with families about care goals.
