AI steps in as your family doctor to manage heart health

Summary
This study is testing a digital tool called a 'Smart Family Doctor' to see if it helps people better manage their health after heart procedures like bypass surgery or stents. The app uses AI to assist with controlling blood pressure, blood sugar, and cholesterol. About 536 adults who have had these procedures and have related health conditions will participate to see if the tool improves their health outcomes and quality of life.
