Summary

This study is testing whether a smartphone app powered by artificial intelligence can help people who have had heart bypass surgery or a stent procedure better control their blood pressure, diabetes, and cholesterol. About 536 adults who already have these conditions will use the app to track their health and get guidance. The goal is to see if the app helps more patients reach their target health numbers compared to standard care.
