Summary
This study is testing whether a program using an AI chatbot health coach and remote vital sign monitoring can help people better manage their interconnected chronic conditions. It aims to reduce serious health events like heart attacks, kidney failure, and hospitalizations. The trial will enroll 300 adults who have diabetes, heart disease, or chronic kidney disease.
