Summary
This study tests whether an AI-powered smartphone app can help people who have had heart bypass surgery or stents better control their blood pressure, diabetes, and cholesterol. About 536 adults who already have these conditions will use the app or receive standard care. The goal is to see if the app helps more patients meet their health targets.
