Summary
This study looks at whether community health workers at senior centers can help adults aged 60 and older improve their blood pressure, physical activity, and eating habits. About 250 participants will either receive health worker support for 4 months or continue with usual services. Researchers will compare the two groups to see if the extra support makes a difference.
