Summary

This study tests whether a video-based telehealth program can help stroke survivors better control their blood pressure after leaving the hospital. About 542 adults who had a stroke or mini-stroke and have high blood pressure will take part. The goal is to see if this remote care model improves blood pressure and reduces racial differences in outcomes.
