Summary
This study tests whether a video-based telehealth program can help stroke survivors better manage their blood pressure after leaving the hospital. About 542 adults who had a stroke or mini-stroke and have high blood pressure will take part. The program uses remote check-ins and education to improve blood pressure control and reduce racial differences in outcomes.
