Summary
This study tests whether a virtual world-based cardiac rehabilitation program is feasible for heart patients. Five participants will use an online program to improve their cardiovascular health, measured by the American Heart Association's Life's Essential 8 score. The goal is to see if this approach can reduce hospital readmissions and improve outcomes.
