Summary

This study tests whether using a home blood pressure monitor paired with a smartphone, plus support from a case manager, can better control blood pressure in people with chronic kidney disease who live in remote areas. About 146 participants in northern Alberta will either use the monitor with usual care or with a case manager who adjusts medications. The goal is to see if this approach lowers blood pressure and helps prevent kidney failure.
