Remote BP monitoring plus case manager tested for kidney patients in rural canada
NCT ID NCT04098354
First seen Oct 31, 2025 · Last updated Jun 20, 2026 · Updated 32 times
Summary
This study tests whether using a home blood pressure monitor paired with a smartphone, plus support from a case manager, can better control blood pressure in people with chronic kidney disease who live in remote areas. About 146 participants in northern Alberta will either use the monitor with usual care or with a case manager who adjusts medications. The goal is to see if this approach lowers blood pressure and helps prevent kidney failure.

What this could mean
Our plain-language read of the trial. This is informational only — not medical advice or a prediction.
Active substance
home blood pressure telemonitoring device and case management
What this could lead to
If successful, this could show that combining home BP monitoring with a case manager improves blood pressure control and reduces kidney disease progression in remote communities.
What could go wrong
This is a small, early-stage study (146 participants) with only 12 months of follow-up. It requires a smartphone and internet, which may limit who can benefit. The intervention may not prove better than usual care.
