Summary
This study will test a home monitoring program called EPOCA for 250 adults recovering from colorectal or weight-loss surgery. The program uses connected devices, video calls, and a care team to support patients at home after early discharge. The goal is to see if this approach can safely reduce time spent in the hospital and prevent readmissions.
