Summary
This pilot study will test a 12-week telehealth program for frail adults aged 65 and older who have just left the hospital. Participants will receive weekly coaching sessions and use activity sensors to track their progress. The goal is to see if the program improves physical fitness, mood, and daily living skills, and reduces hospital readmissions.
