Summary

This study tests whether a telehealth coaching program led by community health workers can help South Asian patients better control their diabetes and high blood pressure. About 450 adults with both conditions will either receive the coaching or usual care. The main goal is to see if more patients achieve healthy blood pressure levels after six months.
