Summary

This study tests whether a telehealth coaching program led by community health workers can help South Asian patients with type 2 diabetes and high blood pressure better control their conditions. About 450 adults will either receive the coaching or usual care. The main goal is to see if more patients achieve healthy blood pressure levels after six months.
