Summary
This study is testing whether a telehealth program can help prevent older adults from falling again after they visit the emergency room for a fall. Participants will have video calls with a physical therapist and a pharmacist while still in the ER, followed by phone check-ins for six months. The goal is to see if this remote support is practical and reduces the chance of another fall.
