Summary
This study is testing whether a telehealth program can help prevent repeat falls in older adults who visit the emergency room after a fall. Participants receive a remote physical therapy and pharmacy consultation while in the ER, followed by phone check-ins for six months. The main goal is to see if this convenient, remote approach is practical and effective at reducing future falls.
