Summary

This study looks at whether referring lonely older adults with diabetes to a social worker can help lower their blood sugar (A1C) and reduce loneliness. About 90 seniors will either meet with a social worker or continue their usual care. Researchers will check A1C from medical records and use a short loneliness survey at the start and after 6 months.
