Summary
This study is testing if 'social prescribing' can help women with heart failure take their medications more consistently and feel better. Social prescribing connects patients with a 'link worker' who helps them find community support for needs like transportation, housing, or loneliness. Researchers will enroll 200 women who face social challenges to see if this personalized support improves medication adherence and quality of life compared to standard care.
