Summary
This study tests whether a smartphone app can help doctors and young adults with chronic kidney disease (CKD) follow the latest treatment guidelines. The app provides reminders, education, and care recommendations to delay kidney disease progression. About 80 participants aged 16-29 with CKD will use the app, and researchers will check if their care aligns with KDIGO 2024 guidelines.
